The service request document is where all the details of the request are filled in. It consists of three tabs.
BASIC INFORMATIONS

At the very top, there is a request completion timer—it counts the days from the date in the REQUEST DATE field.
In the REPORTING PARTY DATA section, there is the contractor’s name and address, the reporting person’s details, and request specifics: Number, Request Date and Completion Date, Responsible Person, Status, Mode, Type and Category of Execution, Tags, and Informed Persons.

In the first tab, you can also find the PREMISES DATA section, which includes the address of the premises related to the service request, a space for attachments, and an additional description.

DOCUMENT DETAILS

This tab contains the SERVICE REQUEST CONTRATOR section, with executor details: name, address, TAX number, details of the performing person (first and last name, email, and phone number), and dates: expected response date, scheduled inspection date, and scheduled defect removal date.
